Police have released the name of a driver who died Monday in a crash on Wolf Road in Colonie.

The crash happened around 1:45 p.m. just south of Sand Creek Road. Colonie Police said Jose Holguin, 45, of Schenectady, was driving north in his Toyota Corolla, when he crossed over into the southbound lanes and collided with Honda Odyssey.

Holguin was pronounced dead at the scene.

The investigation is ongoing, but police said early indications are that Holguin may have had a medical event before the crash.

The other driver, a 42-year-old man from Colonie, was injured and taken to the hospital.
