The National Transportation Safety Board has released its preliminary report on the bus crash that killed two people and injured others near Orange County last month.

The crash took place on Sept. 21. A 2014 Prevost motorcoach operated by Regency Transportation was traveling on Interstate 84 carrying 40 high school students and four adults from Farmingdale on their way to a band camp in Pennsylvania.

At about 1:12 p.m. near Wawayanda, the bus moved out of the right lane of the four-lane divided highway, crossed the left lane and shoulder before leaving the road.

The motorcoach then struck and penetrated a roadside cable barrier and traveled down the depressed earthen median. It came to rest on its left side at the bottom of the depression, according to a news release.

Several occupants were ejected from the motorcoach, including the driver and the two front-row adult occupants. The occupants of the front row died, and the driver was seriously injured. Other people were transported to area hospitals with various injuries.

The report is preliminary and does not state a cause for the crash. The incident remains under investigation.  Initial reports had suggested a faulty front tire was to blame.
